Permission statement

By submitting, you hereby grant permission to Fresenius Kabi USA, LLC and its affiliates and designated third parties (collectively, “Fresenius Kabi”) the irrevocable, perpetual, unlimited, worldwide right to use any and all photos, images, text, and any other content or materials submitted (collectively, the “Story Content”) for any purpose, and to permit any third party to do so, and, without limiting the foregoing, you agree that Fresenius Kabi and any third party permitted by Fresenius Kabi may use, copy, publish, distribute, reproduce, create derivative works from, edit, add to, and modify such Story Content, and you waive any right to inspect any media or other materials (in any format) that may incorporate the Story Content. You also grant Fresenius Kabi the irrevocable, perpetual, unlimited, worldwide right to use your name, likeness, and any other personally-identifying information contained in or associated with the Story Content for any purpose, and to permit any third party to do so. You hereby waive any claims of any kind that you may have against Fresenius Kabi (and/or any third party permitted by Fresenius Kabi) that arise out of the collection or use of the Story Content, and you represent and warrant that you have the full authority to grant to Fresenius Kabi all of the rights and permissions set forth above, and that the Story Content does not violate the rights of any third party, including without limitation any third-party copyrights, trademarks, or rights of publicity.
